Early AVN (Stage I and II) needs medications, observation, physical therapy, and sometimes Core decompression surgery. Other treatment options also exist.

Stage III AVN is treated with Total Hip Replacement (THR) if there is collapse of the femoral head or severe pain.

Stage IV AVN (Osteoarthritis) is treated with a THR (Total Hip Replacement).

Treatment is customised after assessing each patient in detail.

For THR, Dr. Pispati uses Smith & Nephew, Zimmer, Biomet, Maxx (Merill), Stryker or DePuy.

For TKR, he uses Zimmer (Nexgen/Gender), Depuy, Smith & Nephew, or Maxx.

Each patient is analysed in detail and based on various hip parameters of the patient, a hip prosthesis is chosen – customised hip surgery.

We use the the semitendinosus (hamstring tendon) and sometimes the gracilis to reconstruct a torn ACL with anatomic tunnels. This gives the least pain, quickest recovery and the finest scar that heals well. In certain patients we use the peroneus longus or quadriceps graft. Our aim is to enable the patient to return to rapid physiotherapy at the earliest and thus enable the athlete to return to sport at the optimum time.

A Physiotherapist needs to attend to you daily for about 3 weeks after surgery. Please sit on chairs or sofas or use beds that have a minimum height of atleast 18 inches (ideally 20 inches) from the ground. If you are overweight, please use an adjustable height chair commode for 2 months to sit on for toilet purposes (minimum 18-20 inches height). If you are not overweight, you may use a "toilet seat raise" to raise the height of your toilet to atleast 18 inches (this is available at all leading Chemist / Pharmacy shops). Do not bend forward to lift things up from the floor – use a special device called “Helping Hand” for this.

Since knee and hip implants are made of metal, there’s a chance they could set off metal detectors. Patients have reported mixed experiences at airports: some detectors go off and some don’t. You may be provided with a special card to keep in your wallet explaining that you have a knee implant. Alternatively, all you need to do is carry a picture of your hospital discharge summary when travelling.

The amount of knee bending you regain after surgery depends on the amount of knee bending you had before surgery and the amount of hard work and knee exercise you do after surgery. If you knee could bend fully just prior to having surgery, there is a good chance that your knee will bend fully after surgery and you may be able to kneel and sit cross legged 3 months after surgery (be aware, though, that kneeling and cross legged sitting put higher loads on the knee prosthesis and potentially cause earlier loosening). If your knee was very stiff before surgery and you could hardly bend it, then surgery will help improve movement, but you may not regain full knee bending – this is why surgery needs to be done in good time before the knee gets very stiff and should not be unnecessarily delayed.

For Jaslok Hospital appointments, kindly arrive 30 minutes before appointment time. Due to our busy schedule and yours, we request you to so kindly arrive ahead of time. Please arrange your previous x rays, reports and investigations in serial order, with hard copy printouts of the same. Your mobile phones need to be in silent mode or switched off to allow us give you the best consultation.

A video consult needs a higher level of preparation from the patient’s side – all previous relevant doctor consultation notes, investigation reports and a typed history from the patient stating the problem with duration etc, should be kept ready in a pdf file to be sent to us prior to the consultation along with relevant x ray and other films.

Video consults also need higher judgement from our side as the patient is not available to physically examine in person, hence should be followed by an in-person examination to confirm the findings and suggested treatment.
